Patient's Rights and Responsibilities
Each patient has the right to:

Be treated with dignity, respect and consideration. |

Be treated in a clean and safe environment. |
|
|
Receive complete and current information regarding his/her diagnosis, treatment and prognosis in terms he/she can understand. |
|
|
Receive service regardless of age, sex, social status, political belief, disability or diagnosis. |
|
|
Know by name the qualified practitioner responsible for the provision of care. |
|
|
Privacy and confidentiality regarding his/her personal health information. |
|
|
Know the fees for service and policy concerning payment of fees. |
|
|
Participate in decisions about the intensity and scope of treatment, including the establishment of goals and expected outcomes in care. |
|
|
Know the process for resolving patient complaints in a timely manner. |
|
|
Receive information on Independence Prosthetics - Orthotics, Inc.'s policies for receiving, reviewing and resolving customer complaints. |
|
|
Receive reasonable coordination and continuity of care. |
|
|
Be informed of the policy for after-hour and emergency coverage. |
|
|
Participate in feedback assessment within two months following the provision of the device. |
The patient has a responsibility to:

Provide accurate and complete information concerning his/her medical history. |
|
|
Notify the staff when he/she are unable to keep an appointment. |
|
|
Be considerate of the rights of other patients and Independence Prosthetics - Orthotics, Inc. staff. |
|
|
Read, understand, and follow his/her patient care, use and maintenance instructions regarding care. |
|
|
Inform Independence Prosthetics - Orthotics, Inc. if he/she does not understand instructions on care. |
|
|
Assure the financial obligations of his/her services provided are fulfilled promptly according to policy.
